The ProForma generally provides more Customs-specific information than is usually contained on most typical commercial invoices. The vast majority of shipments from Canada to the USA are therefore accounted for using this document.

NAFTA Certificate of Origin
Required to claim preferential NAFTA tariff treatment (including relief from the U.S. Customs Merchandise Processing Fee) for commercial shipments to the USA valued at =>$2,500 USD. It is strongly recommended that you view or print the Completion Instructions (you can find these on page 2 of the form) prior to filling out the NAFTA Certificate. Once completed online, you must then print, sign and date the Certificate.

You do not require a formal Nafta Certificate of Origin for a commercial importation of a good whose value does not exceed US$2,500, provided that, unless waived by the port director, the producer, exporter, importer or authorized agent includes on, or attaches to, the invoice or other document accompanying the shipment the attached signed statement.

If the port director determines that an importation described in the paragraph above forms part of a series of importations that may reasonably be considered to have been undertaken or arranged for the purpose of avoiding a certification requirement set forth in this part, the port director shall notify the importer in writing that for that importation the importer must have in his possession a valid Certificate of Origin to support the claim for preferential tariff treatment. The importer shall have 30 calendar days from the date of the written notice to obtain a valid Certificate, and a failure to timely obtain the Certificate will result in denial of the claim for preferential tariff treatment. A "series of importations" means two or more entries covering goods arriving on the same day from the same exporter and consigned to the same person.

Required for reporting vehicles under five tons - ferry, train, car, vehicle, etc.

This form is supplemental to the commercial invoice and includes additional data elements required by U.S. FDA to meet specific import requirements mandated by the Bioterrorism Act.

Certification regarding the origin and condition of wooden pallets, crating, dunnage, and other wooden packing material imported into the United States from China.

Declaration required by the U.S. Animal and Plant Health inspection Service (APHIS) for the Importation of animals, animal Semen, animal embryos, birds, poultry, or hatching eggs.

Must be submitted to U.S. Customs for each motor vehicle (including motorcycles, etc.) and specified motor vehicle engines, under certain circumstances, that are subject to Federal air pollution regulations.
